Education : MBBS - Medicine , post Graduation Not Required  Job Description : 1. Prescribed application forms can be obtained from the Office of Sri Jayadeva Institute of Cardiology, Bannerghatta Road, Bangalore-560 069
during working; hours from 9.30 AM to 4.30 PM either in person or through post by sending a crossed Indian Postal Order or D.D. of Rs. 250/- for SC/ST and Rs. 500/- for others, drawn in favour of The Director, Sri Jayadeva Institute of Cardiology; Bangalore.
2. Candidates desiring to have the application form by post shall send a self addressed stamped envelope of size 27 V2 x 12 V2 cms with a postage stamp of the value of Rs. 30/- for sending the application by Registered Post, acknowledgement due.
3. Relaxation of Age Limit: - The maximum age limit for appointment shall be deemed to be enhanced in case of candidate who is or was holding a post under the Government or a local authority or a coporation established by the Government and owned or controlled by the Government or a Society Registered under the Societies Registration Act, 1960 by the number of years during which he is or was holding such post or 10 years whichever is less and also in cases as prescribed in Karnataka Civil Service (General Recruitment Rules, 1977).
4. Employees of State Government / Central Govt. / Public Sector undertaking / Society registered under the Societies Registration Act 1960 and other Institutuion shall send the application in the prescribed form through their employer to reach this office on or before the last date fixed.
5. Self attested photo copy of certificates indicating the age, Educational qualification, Marks Cards, experience shall be enclosed.
6. Self attested photo copy of caste certificate indicating SC/ST. Cat.l., HA, IIB, MA & MB shall be enclosed.
7. Selected candidates will be governed by the Institute / Government Rules and regulations.
8. The candidate on appointment will be on probation for a period of 2 years.
9. The number of posts notified may vary as per the requirements.
10. The decision of the Selection Committee in the matter of selection is final.
11. The envelope containing the completed application should be superscribed in the block letters as Application for the post of _ _ _ _ _ _ _ _ _ _ _ _ _ _
